what is it? a kids' club on the pupil free day
who's it for? anyone in kindergarten to Year 6
where? St Annes Ryde Anglican Church, corner Church and Gowrie Sts, Ryde.
how long? 9am to 3pm
how much? $10 per child, $25 per family. Morning tea and lunch included. Please pay on the day by cash, cheque or credit card.
Indemnity
please read this indemnity as it is a requirement of attendance. A copy will be available on the day for you to sign.
My signature below indicates my willingness to permit my child to participate fully in all the activities of CASA, In the case of a medical emergency, I hereby give permission for the necessary treatment of my child as named. I agree to pay all such doctor, ambulance and hospital fees incurred on behalf of my child. I understand that every effort will be made to contact me prior to instituting such procedures.
